Providing in-home care services to those in need

The same quality care patients and families have come to trust from Prisma Health is available beyond the walls of our hospitals and providers.

Prisma Health HomeCare provides intermittent health care services in the home in Richland and Lexington counties. We provide in-home health care services for infants, children, adults and senior adults recovering from illness or injury.

Our services

  • Skilled nursing services – general, medical, post-surgical care and various specialty programs
  • Home health aide services – certified aides provide personal care services
  • Dietary consultation – by a registered dietitian
  • Social work services – assess the emotional and social needs of patient and family, assist with coping skills, and identify the appropriate community services and resources
  • Chaplaincy services – available upon request
  • Physical therapy  – strengthening the overall body and improving walking while teaching the caregiver how to assist the patient in their rehabilitation program
  • Occupational therapy – retraining of arms and hands to increase independence in activities of daily living
  • Speech therapy – maximize patient's ability to communicate in addition to working with patients with swallowing disorders
  • Arrangements can be made for supplies, durable medical equipment and pharmaceuticals when necessary.

Specialty programs

  • Diabetic programs – instruction in insulin administration, glucose monitoring, diabetic diet, wound care and lower extremity care
  • Neonatal and pediatric services – pediatric nurses and physical, occupational and speech therapists provide care for the specialized needs of newborns and children with a broad range of diagnoses
  • Women's health and maternity services – maternal/child nurse provides care for women with pregnancy-induced diagnoses and for women needing post-surgical care
  • Wound care/ostomy – enterostomal therapy nurse provides treatment and consultation on ostomy care and wound healing
  • Infusion services – specially trained nurses care for patients receiving chemotherapy, antibiotics, hydration and nutritionals
  • PATH (Planned Approach to HomeCare) – PATH patients have a potentially life-limiting illness and may choose to continue to receive curative treatment

Home Health services are covered under Medicare and Medicaid, at no cost to the patient or family.
Services also are covered by HMOs, PPOs, private payment, sliding scale fees and private insurance plans. We will be happy to verify insurance coverage and ensure you meet any criteria set by Medicare, Medicaid or your insurance.

Medicare and Medicaid guidelines require:

  • A physician's order and a skilled nursing need in addition to homebound status
  • A skilled need that requires the attention of a professional such as a nurse or a physical, occupational or speech therapist
  • Homebound does not mean "housebound" but the patient must require assistance to leave the home

Prisma Health HomeCare is a non-profit organization serving Richland and Lexington counties since 1986. Our program is licensed by the South Carolina Department of Health and Environmental Control (DHEC), accredited by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO), and is Medicare and Medicaid Certified. In addition, we are members of the National and State Home Care Associations.
