Community Health Needs Assessment

Focusing on our community

A community health needs assessment is designed to help improve the overall health and wellness of communities by identifying, prioritizing and implementing action plans to address community needs.

Required with the passage of the Affordable Care Act, it includes findings from research, interviews, focus groups and surveys with community members, local elected officials, major employers and community organizations. The results of this needs assessment is used to guide and expand our efforts in providing services to the community that we serve.

Action plans

Prisma Health is committed to using this information in collaboration with our partners to respond to our community’s priorities. Action plans for Lexington and Richland counties and Sumter County were developed in early 2017. The results of those plans also are available. All actions will be completed by the end of FY 2019 (Sept. 30, 2019).
