All patient care information is regarded as confidential and available only to authorized users.

Medical records will be obtained from other health facilities when requested by the physician and upon written authorization of the patient. Patients may have access to their completed medical records upon proper written request.

Birth Certificates

Palmetto Health Baptist cannot issue birth certificates. To obtain a copy of your baby’s birth certificate, you will need to send an $12 fee for the first copy and $3 for any additional copies requested at the same time to Richland County's Vital Records Office. The Vital Records Office staff asks that you wait two months from your baby’s birth before requesting the birth certificate.

You may obtain the certificate in person, or by submitting a written letter with the following information...

1. Name of child
2. Date of child's birth
3. Mother's MAIDEN name
4. Father's name if mother is married or if a paternity acknowledgement (PA) was done.
5. Current mailing address of parent(s).
6. Signature of mother or father requesting certificate, and date of signature.
