Birth Certificates

While in the hospital you will be given a birth certificate worksheet with the Social Security Request form from South Carolina Department of Health and Environmental Control (DHEC). You will need to complete this information before you leave the hospital and return it to your nurse. Palmetto Health hospitals cannot issue birth certificates. To obtain a copy of your baby’s birth certificate, you will need to send a $12 fee for the first copy and $3 for any additional copies requested at the same time to the State Vital Records Office. The Vital Records Office staff asks that you wait two weeks from your baby’s birth before requesting the birth certificate. You may obtain the Birth Certificate in person after two weeks, or by mailing the birth certificate application you receive at the time of discharge.
