Office Policies
Please take a moment to review our office policies. Plan to arrive 15 minutes before your scheduled appointment to complete necessary paperwork. To avoid delays, it is advised that you confirm our participation with your health plan before arrival. Please bring your insurance card to ALL visits. To keep your health care costs down, please bring a copy of your health plan's drug formulary so less expensive medication options may be selected for you.
Appointments: Patients are seen by appointment only. The availability of appointments is based on our schedule and medical urgency. We make every attempt to accommodate your needs.
After Hours Emergency Care: There is a physician on-call each night and on weekends. Please be assured your care will be in the best of hands.
Check-In Procedures: In efforts to best serve you, we ask that you bring your current insurance card and a list of any medications you are taking. Please arrive 15 minutes prior to your appointment time to complete necessary forms.
Cancellations: We ask that you notify us 24 hours in advance if you need to cancel an appointment. Failure to do so results in an inconvenience for you and for our schedule.
Insurance and Payment: Current insurance cards are required for your visit. Applicable co-pays and deductibles are collected at the time of service. We accept cash, checks, VISA and MasterCard. We do accept most major insurance plans. Your insurance coverage is a contract between you and the carrier. It is the responsibility of each patient to know what is covered under the terms of her plan.
Prescription Refills: Please allow 48 hours to refill prescription requests. Requests for pain medications are not authorized on weekends or by the on-call physician.
