In an effort to improve communication with our patients, we are excited to implement an integrated patient portal. Now you can have direct access to your records, including your medication list, lab results and medical issues list. There is no charge or fee for this service.
Click the link below to access your chart, review your lab results, request an appointment, request a refill, or ask about your last visit. If you already have a log-in/password, click on "Sign in with Cerner Health" at the TOP RIGHT of the next page. If you received a printout from our office with an access code, click on "Get Started" at the right side of the next page.

If you do not have a username/password, please send an e-mail to TwelveMile@palmettohealth.org. Please ask for patient portal access and include your name, date of birth and e-mail address in the message.
