Individuals accessing services through Palmetto Health Community Health may receive a survey in the mail asking how well services were performed.
 
You will be asked to rank our services on a scale from 1 to 5:

5= A (very good)
4 = C (good)
3= F (fair)
2= F (poor)
1= F (very poor)
 
Please provide your honest feedback. We value your response and appreciate any suggestions, which can be written in the comment section.
 
Upon completing and returning the survey, you will become eligible to win a $100 Visa gift card. Drawings will be held every three months.
