Advance Directives

In compliance with the Patient Self-Determination Act of 1990, Palmetto Health is required to ask all adult patients if they have an advance directive, such as a living will, healthcare power of attorney or psychiatric advance directive. Advance directives are documents that allow you to provide instructions in advance regarding the kind of care you want should you become unable to communicate your wishes to your caregivers.

If you have an advance directive, you will be asked to provide the original document so that it can be copied and placed in your medical chart. If you do not have an advance directive and want one, an Admissions representative can provide you with information. If you want to develop an advance directive while in the hospital, someone from Patient Relations (Guest Services) can assist you.
