Parkridge Surgery Center requests that you pre-register prior to your surgery. Pre-registration enables us to verify your insurance benefits, obtain pertinent medical information including your medical history as well as any medications you currently take.
Pre-registration information will be gathered by your surgeon’s office when your surgery is scheduled.
Your surgeon or the anesthesiologist may require pre-op testing. If so, he or she will specify where the testing is to be done. We have a radiology department and lab drawing facility — Parkridge Imaging Center & Laboratory — in our building.
We will verify pre-registration information by phone at least two to seven days prior to the date of your surgery. During this phone call, we will ask you questions about your medical history.
We do not honor Advanced Directives. If you are interested in Advanced Directives, we will be more than happy to refer you to OMSBUDSOM at (803) 734-5049.
If custody or guardianship of a patient has been awarded by a court of law, original court papers must be presented prior to surgery.
