News ReleasesMaintain a List of your Medications for Patient Safety11/24/2009 As part of our ongoing effort to provide the safest care to our patients, Palmetto Health encourages community members to actively prepare for their next scheduled or unscheduled visit to the hospital or doctor’s office. Maintain a list of all the medications you are taking, and be prepared to share this information with the doctors and nurses caring for you. Be sure your list includes: all prescribed medications Carry the list with you each time you visit your doctor, a pharmacy or one of our hospitals. Doing so helps avoid mediation errors caused by omission, duplication, dosing errors or drug interactions. So ask your healthcare provider for a "universal medication form," available at Palmetto Health hospitals. Or, you can download a copy of our Medicine Record List or Formulario Universal De Medicamentos. Keeping up with your medications (and documenting allergies or reactions you've had to medicine in the past) helps ensure your safety. |
