Family Practice physicians are trained to treat the whole person, physically and emotionally. They provide comprehensive medical care to you and your family. Your family physician will get to know you and your medical history to personalize your care.
About Our Practice
Welcome to Northeast Family Practice. Our office was founded in 1988 in Northeast Columbia. Northeast Family Practice is consistently ranked in the 99th percentile for patient satisfaction.
Our practice is recognized by the National Committee for Quality Assurance (NCQA) as a Patient-Centered Medical Home (PCMH). The Patient-Centered Medical Home is a health care setting that emphasizes partnerships between individual patients and their personal physicians, and when appropriate, the patients’ families.
Meet Our Providers
Melissa Boylan, MD is board certified in family medicine. She received her undergraduate degree from the University of Notre Dame and her medical degree from Georgetown University School of Medicine. Dr. Boylan completed her family medicine residency at Palmetto Health Richland.
Shagufta Chaudhry, MD, is board certified in family medicine. Dr. Chaudhry earned her medical degree from Rawalpindi Medical College, Pakistan. She completed an OB/GYN residency at Rawalpindi General Hospital, Pakistan, followed by a family medicine residency at the University of Pittsburgh Medical Center in McKeesport, Pa. Her special interests include women’s health, preventive medicine, child health and development, and geriatric health.
Rodney Fitzgibbon, II, MD is board certified in family medicine. He is a graduate of University of South Carolina undergraduate school and medical school. His internship and residency were completed at Richland Memorial Hospital (now Palmetto Health Richland).

For your first visit, please arrive 15 minutes prior to your appointment time to complete paperwork. You will need to bring your insurance card; picture identification (such as a driver’s license); and any medications you are currently taking.
The forms below may be printed and completed prior to your arrival. Please bring a copy to your visit.
If an appointment needs to be cancelled, please call at least 24 hours before the scheduled appointment time.
Prescriptions and Refills
Please allow 48 hours for prescription refill requests. You may contact our office if you need additional information about prescriptions.
Fees and Insurance
We accept most insurance plans. If you have a question about our fees or your bill, please call our Physician Practice Billing department at 803-296-7306 and we will assist you. Payment is due when services are rendered. For your convenience we accept cash, checks and credit cards. It is your responsibility to fully understand your coverage benefits and to verify that we are listed as approved providers prior to scheduling an appointment with us.
We are conveniently located at the intersection of Two Notch Road and Interstate 77. From Two Notch Road, turn onto Daulton Drive (at the IHOP), and make an immediate right onto Blarney Drive. We are located in the third building on the left, on the second floor, Suite 209.
